The Senior Impact Program is designed to improve access to community based integrated behavioral health and primary care services to Marion and Johnson County residents over the age of 60. Symptoms of depression are often overlooked or ignored in the older adult population, yet depression is often linked with chronic medical conditions, chronic pain, shrinking social network, or loss of mobility. Depression is treatable. Integrated service planning provide consumers with a complete picture.
• FREE In-home mental health and physical health screening assessments
• In-home, evidenced-based, counseling and medication assessments for people diagnosed with depression
• In-home and community-based care management services, as needed
• Primary health care linkages and ongoing coordination with providers
• Medical services from WindRose Health Network
• Linkages with other community services and assistance with transportation, as needed
